PIP form question 1: your condition
This question is about your condition, medication and treatments. You don’t need to write about how your condition affects you yet, this is covered in questions 3 -15. Below we look at each section of PIP question 1 in more detail, including sample answers.
Question 1a
Question 1a asks you about any health conditions or disabilities you have, and the approximate date they started. You can just write the year if you’re not sure of the exact date. For example:

Question 1b
This section asks you about any medication or tablets you’re taking or will be taking, and the dose. You should include:
- over-the-counter medication
- prescription medication
- medication you should take but can’t because of side effects or another medication
It’s best to include all medications you take, even if you’re not sure it’s relevant. Include how often you take each one, the strength and dose. Check the patient information leaflet if you’re not sure on the strength. For example: Paracetamol, 2 x 500mg tablets, 4 times a day.
If you take a medication to prevent the side effects of another medication, include this too. For example: I take a painkiller for arthritis, but this makes me feel sick. My doctor also prescribed an anti-sickness medicine to take alongside the painkiller.
If you’ve got a printed prescription, attach it to your PIP form when you send it. Write your national insurance number and name on each page.
Question 1c
This question asks you about any treatments, therapies or operations you’ve had, are currently having or have planned. You should include:
- how often your therapy or treatment takes place
- when your therapy or treatment started, or when it’s due to start
- any planned operation dates
- NHS and private treatments, operations or therapies
For example: I have a physiotherapy home visit once a week. This started in January 2023.
I have a knee arthroscopy planned for 15/12/2024 with Dr Majeed at Northwich General Hospital.
